GM FINAL SHORT CASE  JULY 31,2022 This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patients clinical problems with collective current best evidence based inputs. July 31,2022 A 55 year old male patient farmer by occupation presented to the casualty with chief complaints of altered sensorium since 1day and history of fever since 4 days. HISTORY OF PRESENT ILLNESS :  Patient was apparently asymptomatic 4 days ago,then he developed high grade fever with no diurnal radiation and is associated with chills. No h/o cough and enteric symptoms. Attenders present there suggest that there was h/o stoppage of OHA for 3 days and there was decreased intake of food since he has fever.
