History of present illness:   Patient was apparently asymptomatic 35days back, then he developed shortness of breath for which he went to a local hospital after further investigation he was diagnosed with chronic kidney disease and underwent 4 sessions of dialysis.Then referred I/V/O dialysis. H/o present illness- Patient was apparently asymptomatic 4 months back and then he had a burning type of pain in abdomen(epigastrium) radiating to the chest and then with frequent vomitings, 2-3 times per day after taking meals. He had a burning micturition and while passing stools.
